Eligibility
Individuals of all ages with a developmental and/or intellectual disability documented as primary diagnosis, and not enrolled in any Medicaid or home and community-based services waiver progrom or receiving any services funded by the DDA.
Required documents
Proof of applicant identity or parent/legal guardian identity if applicant is under age 21, proof of disability diagnosis, and proof of residence.
